At Valley View Senior Village, we offer an array of rehabilitation services and 24-hour skilled nursing care for our short term care patients.
The ultimate goal is to form and follow a care plan that is designed to get a patient back to the "baseline" level of functioning before the accident, surgery, or hospital stay occurred.
At Valley View Senior Village, our Resident Care Coordinator works with residents, family members, physicians, nursing staff, therapists, dietitians, and other health providers to craft a care plan tailored to each individual resident. The goal of the care plan is to allow the resident to return home or to a residential care environment.
Valley View Senior Village also offers respite care. This is temporary care provided by a skilled nursing facility so a family member or friend who is the patient's care giver can rest or take some time off. In this situation, a physician will write a referral for the patient to stay for a short duration of time at a skilled nursing facility like Valley View Senior Village.
